Their aim is to help improve people’s health and wellbeing by making local connections between people and the services available in their community. They support patients to identify problems and issues they are experiencing and to talk about what really matters to them and achieve goals by enabling the patient to identify and access the relevant resources or services in the local community.They can point you in the right direction and help you find local services on a range of non-medical topics to meet your needs and interests, including:
